Instructions for Admissions Application – Via Surface Mail or Fax

  1. Download and open the long-term or short-term application form. This requires Adobe Reader.
  2. Print out the application form, complete, and mail to: Maplewood Nursing & Rehabilitation, 100 Daniel Drive Webster, New York 14580, Attn: Lynn Pierce – Director of Social Services; or fax to (585) 872-0834; you are always welcome to drop off your application at our Business Office.

Instructions for Admissions Application – Via Email Attachment

  1. Download and open the long-term or short-term application form. This requires Adobe Reader.
  2. While in Adobe Reader, complete the application form by typing into the supplied fields.
  3. When your application is complete, click the Save button at the end of the application form.
  4. After clicking the Save button, select the location on your computer that you would like to save the completed application form.
  5. After the form is saved, compose a new email message in your email program, and attach the saved application form to your email message.
  6. Send the email message with application form attached to Lynn Pierce – Director of Social Services at: information@maplewoodnursinghome.com.
